The Passport has Launched!
The Neurological Alliance Tees Valley, Durham and North Yorkshire has launched a new product to support UK Carers Week, 10th-16th June.
The Neuro Information Passport can be used by anyone who lives with or cares for someone with a Neurological condition. The Passport helps the individual keep track of key information such as medication and symptoms so they don’t forget anything when attending condition related appointments such as with their consultant or GP.
Developed in partnership with service users, The Neuro Information Passport is being trialled before being rolled out to target some of the 36,000 in the Tees Valley, Durham and North Yorkshire area with a Neurological condition.
